Job Description

The Nurse Practitioner (NP), Oncology, will provide specialized and advanced nursing care to meet the unique needs of oncology patients in the Oncology Clinic and inpatient unit, in collaboration and partnership with the physicians and other members of the interprofessional team. The NP provides clinical and strategic leadership within the program, and is responsible for the promotion and enhancement of Oncology care across the health care continuum. The NP will deliver and develop excellent patient care through the roles of practitioner, consultant, educator, and leader. By utilizing theoretical and clinical expertise in this specialty area, the NP delivers advanced practice nursing to patients and families with complex situations within the clinic and in-patient setting, demonstrating a strong understanding of oncological disease processes and care options.

Reporting to the Senior Clinical Program Director, Oncology and Endoscopy Program, The successful candidate integrates research into practice to ensure best practice standards are maintained and incorporated into clinical initiatives. The NP contributes to the education and professional development of nursing and the interprofessional team, and to the wider professional community within their area of specialization.  The NP also initiates and leads best practice initiatives that promote improved patient outcomes in their specialty area and/or on a corporate basis. The NP will participate in quality improvement initiatives, and creates, utilizes and/or disseminates new clinical knowledge and strategies to optimize outcomes, experience and system performance.

 

Responsibilities:

The Nurse Practitioner (NP) – Oncology will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for clinical and inpatients within a philosophy of patient-centered care. This will involve health assessment and diagnosis of medical and cognitive problems, developing and implementing a plan of care, providing health counseling and emotional support. The NP will work in collaboration with consulting Oncologists to facilitate early assessment and intervention for patients requiring oncology consultation.

Clinical Practice:

  • Uses theoretical and evidence-based practice to plan, implement and evaluate advanced nursing care for patients within the Oncology program
  • Conducts a thorough health assessment of the patient’s physical, psychosocial, cultural and spiritual needs by completing a patient history and physical examination and documenting these findings in the patient’s record
  • Utilizes the Edmonton Symptom Assessment Scale (ESAS) and Palliative Performance Scale (PPS) to monitor, assess, and evaluate nursing interventions of disease related symptoms and the patient’s response
  • Prescribes and monitors drug therapy and orders and interprets appropriate diagnostic and laboratory tests within the current legislative framework and internally developed and approved authorizing mechanisms such as medical directives
  • Formulates and implements a holistic plan of nursing care in collaboration with the patient, family and the health care team and that complements the medical plan of care
  • Provides ongoing education to patients and family regarding symptom management, medication management, general information about required tests or procedures, and end of life care (where applicable)
  • Reviews relevant quality and performance data and develops clinical practice action plans to create patient and system level improvements

Consultation/Collaboration:

  • Facilitates communication of patient care needs to the individual appropriate team member(s), to the weekly designated Oncologist at daily Oncology rounds, and to the interdepartmental or community services (i.e. Oncology Clinic and/or Supportive Care Clinic) as required
  • Assists patients and families to navigate through the inter-professional team, departments, services and community supports required to coordinate care throughout the patient’s length of stay
  • Act as a nursing resource to access port-a-cath and PICC lines for oncology patients throughout the Health Centre, as required
  • Facilitates referrals and patient transfers to external cancer care facilities and the offices of the corresponding consultants and/or clinicians, including appropriate documentation and summary letters

Leadership:

  • Participates in professional, program and hospital-wide initiatives
  • Leads the introduction and monitoring of key Cancer Care Ontario initiatives
  • Actively promotes professional role through participation in external forums such as the Nurse Practitioner Association of Ontario
  • Provides leadership in the development/revision of practice guidelines, protocols, policies and procedures
  • Acts as a clinical practice mentor and role model and facilitates integration of evidenced-based practice and clinical practice guidelines into practice

Qualifications:

  • Masters Degree in Nursing required.
  • Current registration in the Extended Class with the College of Nurses of Ontario required.
  • Current Certificate of Competence with the College of Nurses of Ontario required.
  • Minimum of 3 years recent experience working with clinic and inpatient Oncology patients preferred. Demonstrated and comprehensive understanding of pain and symptom management related to this patient population required.
  • Certification in Chemotherapy Administration required (deSouza certification preferred).
  • Certified in Oncology Nursing (Canada) (CON(c)) preferred.
  • Experience with Central Venous Access Devices (CVAD’s) and Computerized Ambulatory Drug Delivery Pumps (CADD) required.
  • Excellent clinical skills in the area of nursing with the ability to provide care to a broad spectrum of patients in an inter-professional care setting.
  • Membership in related professional associations required, e.g. (NPAO, RNAO, etc) strongly preferred.
  • Demonstrated evidence of sound organizational, prioritization, and conflict management skills with the ability to work well in stressful situations.
  • Excellent communication, negotiation, teaching and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community.
  • Ability to work well autonomously.
  • Demonstrated excellent oral and written communication skills, knowledge of the principles of adult education.
  • Satisfactory attendance record required.
